Providers with Regence Blueshield, Premeira, Aetna, Uniform, Cigna, First Choice Health, L&I, and Auto accidents.
If you intend to use your insurance benefits, please bring a prescription that has the following 3 things on it: 1) a diagnostic code, 2) # of sessions, and 3) valid start and end date. Your doctor can fax this to us at (206) 782- 8955.
